At Senior Health Primary Care, our providers are committed to making the aging process as healthy and positive as possible by addressing, assessing, and treating the illnesses, ailments, and normal body changes associated with age. Our specialty-trained physicians, called Geriatricians, received dedicated instruction in the heath and wellness of older adults. Each one completed in either Internal Medicine or Family Medicine with an additional 1-2 years training in the medical, social, and psychological issues that concern seniors.

Electronic Medical Records System

Utilizing our state of the art Electronic Medical Record System, each physician has complete patient data (immunizations, allergies, lab results, etc.) at their fingertips to enable them to monitor your health needs more closely. If you call after hours, all physicians “on-call” can access this same information, thus providing excellent, all-inclusive care for our patients 24 hours a day, 7 days a week. This robust Medical Record System also enables our office to electronically transmit prescriptions and refills directly to the pharmacy of your choice.

In addition, our office also has the facilities to take and process X-Rays and to draw most lab work right on site, offering our patients convenient and efficient healthcare.

Services

Senior Health Primary Care is located within the Elliot Senior Health Center.   Please visit the Elliot Senior Health Center home page for detailed information on the coordinated, comprehensive services provided to aging adults.

Weekend After Hours Clinic

Elliot Physician Network primary care providers recently opened a “Weekend After-Hours Clinic” on Saturdays from 10 a.m. to 2 p.m. at Briarwood Primary Care (located near Elliot Hospital at 445 Cypress Street, Suite 5 in Manchester), allowing their patients the flexibility of being seen for acute matters over the weekend. A rotating schedule of on-call EPN providers will determine which doctor a patient sees at the clinic. If necessary, the physician on-call will follow up with the patient’s primary care provider. Patients access the clinic by calling their physician’s office. The Nurse or the on-call provider will discuss the patient’s symptoms and make a referral to the After-Hours Clinic.
